Provides therapy services and treatments to patients referred for occupational therapy. Evaluates patient status, establishes treatment plan of care for each patient. Re-evaluates periodically patients progress and reports/recommends programs of care to referring physician. Provides training of patients/family in care of injury/disease. Prepares reports of patients status. Program of care for 3rd party payers.

EDUCATION/TRAINING/EXPERIENCE:

Knowledge in industrial rehabilitation care, orthopedic, neuro-muscular disease and injury, cardio-vascular, traumatic brain and spinal cord rehabilitation and pain management progress.

LICENSE/CERTIFICATE:

License by national Board of Certification Occupational Therapy, graduate of accredited school of Occupational Therapy and Basic Life Support (BLS).
